Most people start dieting with straightforward eating strategies for achieving their goal: eat less, lose weight, feel better, and live healthier. However, the side effect that often follows years of eating based on externally generated food rules is the increased probability of disconnection from the body’s own hunger and fullness signals. The consequence of this disconnection is that the otherwise automatic gauge associated with hunger and fullness becomes genuinely confused.

Do you regularly finish a meal and realize you had no clear sense during the meal of when you had eaten past the point of comfort? Perhaps you are realising that your ability to distinguish between physical hunger and the urge to eat for other reasons has been compromised? Those disconnections are not personal failings. They are predictable outcomes of regularly overriding the body’s internal signals with the use of external rules for long enough that those internal automatic signals become harder to detect. They have become dormant and less reliant in managing your nutritional health needs.

Picture an innocent newborn. Somehow that newborn knows they need to fuss or cry when they are in need of food. They physically seek food by turning their body towards the food source and stop crying when that source is within reach. Observing closely during feeding, the baby is able to communicate their state, whether hungry or full. They fuss, cry and coordinate various body movements to communicate the message their body is sending.

Take a second look at the image. Can you tell whether this baby is hungry, engaged and genuinely interested in eating, or is he full and not interested? Regardless of your impression, the baby knows. The real point is that the baby is responding to an innate signal that automatically signals his appetite. A reliable, coordinated external response follows. So, what happens when there is a disconnection between us and our innate hunger and fullness? As we grow, the quality of the immediate social and feeding environment we live in determines whether our innate cues are honoured or suppressed in favour of external food rules. What Hunger and Fullness Cues Actually Are

The Role of Interoception

Hunger and fullness cues are part of a broader physiological process called interoception, the body’s ability to sense and interpret internal physical signals. Interoception covers heartbeat, breathing, temperature, pain, and appetite regulation. The nervous system continuously sends signals about the body’s internal state to the brain, which uses that information to regulate behavior, including eating.

How Dieting Disrupts This System

Chronic dietary restriction overrides the reliable interoception system in two distinct ways. Physiologically: consistently eating less than the body needs leads to chronically elevated ghrelin (a hormone in the stomach that helps in signaling the need for food) and reduced leptin (signaling the level of energy in the form of fat) sensitivity. The hunger signal gets louder and more persistent, while the fullness signal becomes less reliable. Repeated cycles of restriction and refeeding compound this effect: research shows that even after weight restoration, formerly restricted individuals can have persistently altered hunger and fullness signals resulting in chaotic eating signals for months or years afterward, as the body’s survival mechanisms prioritize preventing future food scarcity over comfortable appetite regulation.

Psychologically: following external food rules (eating according to a calorie restricted plan, unreasonable meal times or a calorie count rather than internal cues) gradually erodes the habit of consulting internal signals at all. Over time, the brain stops treating hunger and fullness as meaningful information and starts treating meal timing and portion rules as the primary input. After years of this, the internal signals are still there, but noise from external rules has crowded out the path to them.

Losing touch with hunger and fullness cues is not a sign that the body is broken. It is a sign that external rules have been trusted more than internal signals for long enough that rebuilding that trust requires deliberate, consistent and gradual practice.

What Hunger and Fullness Actually Feel Like

Physical Hunger Versus Other Urges to Eat

One of the most practically useful skills in reconnecting with hunger and fullness cues is learning to distinguish physical hunger from the other states that prompt eating. Physical hunger builds gradually over several hours after the last meal. It produces specific physical sensations, including a hollow or empty feeling in the stomach, low energy, difficulty concentrating, and sometimes mild irritability or headache. A range of foods can satisfy physical hunger, not just specific cravings.

Emotional hunger, in contrast, arrives suddenly and often attaches to specific comfort foods. It tends to follow an emotional trigger rather than a time gap since the last meal, and it does not resolve cleanly with eating the way physical hunger does. Distinguishing between the two is not about refusing to eat emotionally. It is about developing enough awareness to make a conscious choice rather than defaulting automatically, which requires being able to tell which state you are actually in. The Link to Eating Disorder Recovery

In clinical eating disorder treatment, professionals consider rebuilding interoceptive awareness and hunger and fullness recognition a core therapeutic goal, not an ancillary one. Stronger interoceptive awareness consistently links to more effective emotion regulation, greater flexibility in tolerating distress, and lower rates of anxiety and depression.

This connection is practically significant for people whose mental health and eating patterns are intertwined. Rebuilding the ability to notice and respond to internal physical signals is not separate from mental health work. It is part of it. Understanding the connection between food and mental health gives useful context for why the body’s internal signals matter not just for eating but for wellbeing more broadly.

How to Start Rebuilding Hunger and Fullness Awareness

For people who have been restricting food for a long time, this phase requires patience and professional guidance from a mental health professional and a registered dietitian experienced in this area. Hunger cues do not normalize overnight. As the research on mindful and structured eating shows, combining consistent meal structure with deliberate attention to internal signals produces faster and more durable reconnection than either approach alone.
